Please find below a Migraine diary which can be easily downloaded and printed off. It may help to suggest to your patients that they fill it in over time so that you and they can develop a better understanding of their migraine. There are two parts:
The patient should fill in the date that each headache/Migraine began and ended, associated symptoms and details of the pain that occurred. They should also detail what treatment or medications they took and any possible triggers for the event.

In addition to the paper diary we have also developed an eTool that can be downloaded and retained on a PC. In a similar way to the paper diary it enables your patients to record the dates of their headache/Migraine and details relating to it. This information can then be printed off as a record for them to bring to discuss with you the next time they have an appointment. This may assist you and them in the understanding of their migraines.
